Applicant must be a qualified professional (not licensed or provisionally licensed).
Minimum of a 4 year degree within Human Services and a minimum of 2 years full time experience required.
The primary responsibilities of the Care Manager are:
The primary responsibilities of the Care Manager are:
- To promote whole-person.
- Integrated care by planning.
- Coordinating, tracking.
- Closing gaps in care monitoring care to individuals, and achieving key goals of the tailored plan related to behavioral health, mental health, and physical health.
- Must be comfortable with direct patient contact, including in-home and community appointments.
- Experience with NC Tailored Care Management is required.
The Care Manager shall report to the Care Manager Supervisor.
This is a hybrid position with some in office.
